Where Did We Come From? Where Are We? Where Are We Going?

It's been two and a half years since this topic was discussed. Since then:

Has the quiz bowl community progressed any in being more welcoming to women players?
Have new issues arisen?
Have old ones been resolved? Are they still prevalent?
What changes need to be made?
